Post-Surgical Stiffness: Why Movement Matters

Stiffness is a common and often frustrating part of recovery after surgery. Whether following joint replacement, ligament reconstruction or soft tissue repair, many people experience difficulty moving the affected joint in the early stages of rehabilitation

While this stiffness can be concerning, it is a normal response to surgery and can be effectively managed with appropriate rehabilitation.
After surgery, swelling, pain and muscle inhibition all contribute to reduced movement. Protective muscle guarding and reduced confidence in using the joint further reinforce stiffness. Left unaddressed, this cycle can lead to persistent restriction and delayed functional recovery.
Early, guided movement plays a critical role in preventing long-term stiffness. Controlled range-of-motion exercises improve circulation, reduce swelling and encourage tissues to adapt to movement again. Importantly, early movement does not compromise surgical repairs when exercises are appropriately selected and progressed.
Prolonged rest or avoidance of movement often worsens stiffness. While protection of healing tissues is essential, complete immobilisation beyond what is medically required can lead to joint capsule tightness and muscle shortening. Physiotherapy helps strike the balance between protection and movement.

Rehabilitation programs are tailored to the type of surgery, tissue healing timelines and individual factors such as age, activity level and pre-operative function. Some discomfort during movement is expected and does not necessarily indicate harm. Understanding this helps patients engage more confidently in their rehabilitation.
Consistency is more important than intensity. Small amounts of regular movement are more effective at improving mobility than infrequent, aggressive stretching. As swelling and pain settle, movement is gradually combined with strengthening to support functional use of the joint.
Addressing stiffness early supports better long-term outcomes. Movement restores confidence, improves function and helps patients return to daily activities more efficiently and safely.
